At Hearts at Home Care, our service users with complex care needs are typically referred to us from an NHS Clinical Commissioning Group (CCG) or from an independent case management company. However, we also work with local authorities and directly with privately funded clients and have been doing so for over 10 years.

Complex home care services support a person who has substantial and ongoing health needs. Complex care is also known as continuing care.
Complex care is a holistic care service that provides support in all aspects of the physical, clinical, cognitive and social needs of our service users. Hearts at Home Care's, care managers offer support with complex medical needs, and our professional care assistants provide a one-to-one care service enabling complex care clients to remain in their own homes with dignity and independence.
Hearts at Home Care offers bespoke complex care services to service users with a range of different needs. From heartfelt companionship and support with light domestic tasks to specialist care and bespoke support for different medical conditions. Our complex home care services are holistic, flexible and beneficial service for those with ongoing needs.
Eligibility for NHS-funded continuing care is normally determined by the local clinical commissioning groups (CCGs).
To be eligible for NHS Complex Healthcare, an individual’s main or primary need must relate to their health. If their primary reason for needing care relates to their health, the NHS is responsible for providing for these care needs. A complex medical condition that requires significant intervention and ongoing support is likely to be assessed as requiring complex care. This also applies to end-of-life (or palliative) care if the condition is deteriorating rapidly.
For those that are not eligible for NHS-funded care, funding may be available through the social care budget which is managed and arranged by local authorities (councils). Social care benefits are ‘means tested’ (i.e. an individual’s personal financial circumstances are assessed) and local authorities are permitted to charge for services they arrange or provide (or ask for a contribution to the cost).
